Mario Nawfal dropped a post this morning (above) that cut through the glossy influencer ads and celebrity weight-loss glow-ups. Nearly 4,000 lawsuits. Stomach paralysis so severe that food sits and rots. Endless vomiting. Feeding tubes shoved down noses in the ER. Sudden permanent blindness. Pancreatitis. Kidney damage. Muscle vanishing so fast some users look like they’ve aged a decade in a year. Lawyers are already tossing around $2 billion figures.
The FDA finally added a gastroparesis warning to the label in October 2025. A little late for the people who already lived it.

One woman in a widely shared clip lays it out without the pharmaceutical marketing filter: muscle melting off the frame, food fermenting in a paralyzed stomach, weeks of nonstop vomiting, teeth suffering from chronic dryness, and the sudden black curtain of permanent vision loss. Others pile on with their own stories of gallbladder removals, hospital stays, and bodies that no longer feel like their own. The platform is full of them right now—raw, unpolished, and a lot more honest than the before-and-after photos that flooded social media for years.
Gastroparesis is the big one in the main federal MDL (nearly 4,000 cases and climbing). The stomach simply stops moving food the way it should. What starts as the famous “Ozempic nausea” can turn into a chronic condition where meals sit for hours or days. Some patients report the literal smell of rotting food coming back up. Ileus, bowel obstruction, and severe dehydration follow close behind. Kidney injury often trails the dehydration. Pancreatitis shows up in the complaints too.

Then there’s the separate vision-loss MDL. Non-arteritic anterior ischemic optic neuropathy—NAION—hits without warning. Sudden, painless, and often permanent. Harvard researchers flagged a several-fold higher risk. European regulators called it a very rare side effect and updated their labels. The U.S. label still lags.
And the muscle loss. Clinical data has shown that a substantial portion of the weight people drop on these drugs is lean mass—sometimes 25 to 40 percent of the total loss. For older users or anyone already low on muscle reserves, that can look like accelerated aging. Strength disappears. The face hollows out. The “Ozempic face” and “Ozempic body” memes weren’t just internet cruelty; they were describing a real trade-off many users didn’t fully understand going in.
None of this means every person who takes semaglutide or tirzepatide ends up in an ER. Plenty of diabetics and obese patients see meaningful benefits under careful medical supervision. Obesity itself is a serious disease with its own brutal list of complications. But the marketing machine sold these shots as a near-risk-free shortcut. The fine print, the delayed label updates, and the growing pile of lawsuits tell a different story.

Video after video of people who took the shot, lost the weight, and then lost a lot more. Stomachs that quit. Eyes that went dark. Muscles that never came back the same.
